Question:
Thanks to the Portfolio Holder for clarifying the means by which patients can access help on their discharge from hospital.
My supplementary question is: What percentage of patients discharged from hospital during the last three months and who required the services of Reablement, were contacted within the specified 2 working days by the Reablement team, or 24 hours by Home First, the timescale within which they undertake to make an initial visit? We are currently updating our reporting systems, following Discharge to Recovery and Assess COVID-19 guidance from Welsh Government.
As reablement can be both a primary and secondary referral, we are unable, without undertaking a manual count, to identify the percentage of people who were contacted within the specified two days by the reablement team. There were 243 primary referrals to reablement within the three month period.
With regards to Home First, this is a service that is managed by PTHB. Below are the percentages of people who were assessed at home by the Home First Team within 2 working days upon receipt of referral. Please note that this is PTHB data, which PTHB have given us permission to share:
In April, 100% of patients were assessed at home within 48 hours. In May, 96.9% of patients were assessed at home within 48 hours. In June, 81.8% of patients were assessed at home within 48 hours.
