Can the Portfolio Holder for Adult Services explain the mechanism which triggers an intervention by the Reablement team with patients returning home following a stay in hospital. Reablement is a joint service between Powys Social Services and Powys Teaching Health Board [PTHB], utilising both Health and Social Care professionals.
The purpose of the Reablement Service is to provide short term support to individuals to retain or regain their independence, at times of change and transition, which promotes their health, well-being, independence, dignity and social inclusion. The service is available to individuals who are over 18 years old and who permanently reside in Powys.
Service users must be medically fit to engage in Reablement and be able to give informed consent to participating in their own unique Reablement journey. Following assessment which must include a “what matters to you conversation “an individual will be eligible for Reablement where it is clear that there is potential for them to achieve their goals and/or to support the person in ascertaining if and what longer term support they may require.
Referral to Reablement is via ASSIST and once screened allocated to the Reablement Teams. Reablement have joint daily screening calls to ensure the person is allocated to the most appropriate assessor, who makes contact in 2 working days. Reablement aim to assess and provide intervention in 5 to 7 working days from first contact for assessment. There are however delays if there is a capacity issue in frontline staff to support, equipment etc which the team endeavour to avoid.
In late 2019 PTHB introduced a new service called Home First to expediate hospital discharge and work intensively with people for up to 10 days with Rehabilitation prior to Reablement intervention. This to date has reduced the referrals to reablement. This service operated 7 days a week between 8am and 6pm and has been continued in its delivery to support the timely transfer of people from hospital to home during the COVID-19 pandemic.
